Our Community Assessment Process
In 2007, Three Rivers District Health Department began implementation of the MAPP (Mobilizing Action through Planning and Partnerships) process. MAPP is a strategic planning tool developed by the National Association of City and County Health Officials (NACCHO) to identify and improve community goals.
As a part of the MAPP process, Three Rivers was the first Health Department in the state of Kentucky to complete the second version National Public Health Performance Standard Program (NPHPSP). The NPHPSP and several other community assessments were conducted to identify public health problems in Carroll, Gallatin, Owen and Pendleton Counties. After identifying problems, community partners in all four counties formed collaborative groups to solve the perceived and projected health and safety related short falls of the communities. While the groups are a collaborative effort of community partners in each county, Three Rivers acts as an organizing agency. The meetings are always open to the public. All opinions, concerns and ideas are welcomed and encouraged.

- The Carroll County Health and Safety Partnership meets at noon on the fourth Tuesday of the month at the Carroll County Cooperative Extension Service Office in Carrollton. For more information about this group, contact Becky Wilson at the Carroll County Health Center at 502-732-6641.
- The Gallatin County Health and Safety Collaboration meets at noon on the third Monday of the month at the Gallatin County Health Center in Warsaw. Contact Melody Stafford at the Administrative Office at 502-484-3412.
- The Owen Wellness Engagement Network (O.W.E.N.) meets at noon on the second Wednesday of the month at the Itron Training Center in Owenton. Call Katie Gilson at the Owen County Health Center at 502-484-5736 for more information.
- The Pendleton County Community Service Alliance (CSA) meetings are at noon on the last Wednesday of the month at the Southside Christian Church in Falmouth. For more information about this group, contact Jim Thaxton at the Pendleton County Health Center at 859-654-6985.
